Not going to go into details, as the topic is fairly complicated and it's pretty well explained here, but:

Left-side mods:
Square – always offence, so there's no choice

Diamond – always defence, so again no choice

Circle – either Health or Protection, for 5pip mods, max primary bonus is 5.88% HP or 23.50% Protection, so the choice is clear. ;) For the healers though, since usually their heal is based on their own max health, you might choose Health instead.

Right-side mods:
Arrow – either offence, defence, HP, protection, speed, accuracy or crit avoidance; For high damage characters you want to have speed here, for tanks protection. That said, there are some situations where it's not true – for example you want Saint Han to go first, so he should have speed here, dispel users might benefit from accuracy here(nothing worse than enemy taunting tank avoiding dispel ;) ) and so on…

Triangle – either offence, defence, HP, protection, crit chance or crit damage; for DPS, crit chance at low gear levels(crit chance rises with gear too, as well as levels and stars) and later crit damage; for tanks protection

Cross – either offence, defence, HP, protection, potency or tenacity; for DPS, offence, for tanks protection, debuffers might benefit from potency(easier to land debuff) some characters might benefit from tenacity(for example to avoid stun and/or ability block).

That's it, pretty much – remember that these are general guidelines, specific characters might actually require something else to work well. Not to mention that this is mostly arena oriented – while it's not bad in other game modes, there are situations where it will be better to mod your character differently, for example, Teebo absolutely needs potency cross instead of protection to help him land that TM reduction on Rancor and so on and so on.

Found this useful to know what kind of mod to get for each type of character, in general terms (like Torn said, there are particular characters that need particular mods):

Also useful there: max values for each type of mod and stat, so you don't go looking for, say, a mod with 8% health, or 40 speed, or whatever.
